Are you looking for a new challenge in a fast-paced, dynamic environment? At Memorial Hermann, Care Management is a multidisciplinary team that includes Nurse Managers, Case Managers, Social Workers, Nurses, Pharmacists, Physicians and other healthcare providers as needed such as Therapists and Clinical Documentation Specialists. The goal of the care management initiative is to better manage care coordination through earlier involvement in patient placement and discharge planning. The aim of care management is to assist staff, patients and doctors in effectively caring for patients throughout the continuum of care.
Who is the Memorial Hermann Care Management team looking for? Individuals with inpatient hospital experience in the following positions:
- Case Managers: This role integrates and coordinates resource utilization management, care facilitation and discharge planning functions.
- Social Workers: This role is responsible for interacting with the interdisciplinary team (including Physicians, Case Managers, Staff Nurses and other members of the care team) to provide services for individuals from at-risk populations.
- Utilization Review Specialists: This role is responsible for working collaboratively and maintaining active communication with physicians, nursing, other members of the multi-disciplinary care team, and payors to effect timely, appropriate patient management.
- Referral Coordinator: The MDS Coordinator (LVN) is responsible for case managing care plans, including rehabilitative programs.
